MEMBERSHIP APPLICATION

Individual members can join OPHA by using the membership form below. Please select the appropriate membership level.

Current members should log-in to the OPHA website (use the box in the upper right-hand corner) to renew.

Corporate members can join or renew their membership by completing the 2022 OPHA Corp Mbr App.docx and submitting it to OPHA by mail, fax or email (opha@opha.net).

Regular - $60.00 (USD) Subscription period: 1 year, on: January 1st Automatic renewal (recurring payments) Any person professionally engaged, or actively employed, in any branch of public health, or any person who is actively interested in public health in Oklahoma.

Regular (2 years) - $100.00 (USD) Subscription period: 2 years, on: January 1st No automatically recurring payments Regular members who receive a discount for renewing for two years.

Community Health Worker - $25.00 (USD) Subscription period: 1 year, on: January 1st Automatic renewal (recurring payments)

Retired - $25.00 (USD) Subscription period: 1 year, on: January 1st Automatic renewal (recurring payments) A retired member is one who was, at one time, engaged in any area of public health either professionally or voluntarily and is no longer engaged or employed in public health full-time.

Student - $25.00 (USD) Subscription period: 1 year, on: January 1st Automatic renewal (recurring payments) A student member is any person attending an accredited two or four-year college or university on a full-time bases.

Organizational - $60.00 (USD) Subscription period: 1 year No automatically recurring payments For employees and students whose organizations are group members with OPHA. If you do not have the discount code associated with your organization, please contact opha@opha.net before submitting your application to avoid being charged.
