OPHA MEMBERSHIP

OPHA membership runs on the calendar year from January 1 through December 31. Current members need to log-in to the website to renew. If your membership has lapsed, you will be required to complete the membership application again.

Corporate Membership


Corporate membership is for: 
  • Businesses agencies and organizations, or health-related sub-units, interested in public health.
  • Organizations with 50 or more full-time employees

How can my business, agency, or organization become a Corporate Member?

Complete the Corporate Member Application Form and submit the form to the address listed. All corporate member applications will be submitted to the OPHA Board for approval at the next occurring board meeting. You will be notified within 30-60 days if your application has been approved.

Membership Types & Dues:

Dues are based on the total number of full-time employees. Corporate membership dues are capped at a maximum of $10,000.
